About the Role
As a Care Coordinator, you’ll be at the forefront of delivering compassionate, client-centred care. Your primary responsibilities will include managing a caseload of clients, coordinating support services, and ensuring that all care plans are aligned with the Aged Care Quality Standards and CHSP guidelines.
You’ll take an active role in conducting assessments and reviews, ensuring that support services meet the evolving needs of each client. This is a fulfilling role where your efforts will make a tangible difference in the lives of older Australians, supporting their independence and well-being.
Key Responsibilities:
- Support clients by coordinating care services that adhere to aged care standards.
- Organise client intakes, conduct assessments, and create care plans that enhance well-being and independence.
- Track client needs and adjust support services when necessary, referring to other services if required.
- Work closely with clients, families, and service providers to ensure high-quality, tailored support.
- Maintain accurate records and meet compliance and reporting requirements.

What We’re Looking For

Skills & Experience:
- Prior experience in aged care, health, or community services
- Knowledge of Aged Care Act and CHSP guidelines (or a willingness to learn)
- Excellent communication and coordination skills
- Empathy, cultural sensitivity, and a commitment to enhancing the lives of older Australians
- Strong documentation, time management, and organisational skills
- Open driver’s license (desirable)
- NDIS Worker Screening Clearance (Yellow Card)
- QLD Blue Card (working with children’s check)
- Relevant qualification in Community Services, Aged Care, or related fields (desirable)
- Australian working rights (with no restrictions)
Your ability to build trusted relationships with clients, families, and service providers will be key to your success in this role.
